In the page edit window, click the "Insert/edit image" button and add an image description. You may mark an image as decorative, but it is best practice to add a description of the image. There are times when Cascade will auto-fill the image description with the filename of the image. This will also cause an accessibility error, so please replace any auto-filled filename with an actual description.
