Participant Authorization Forms
In order to help your programs remain compliant with the Campus Programs for Minors University Rule 24.01.06W1 there are several release forms that need to be completed by program staff and participants. Program Sponsors are required to retain program-related documentation per the System Records Retention Schedule and the Office of General Counsel. These forms are collected in the registration software used by the WTAMU campus. Please reach out to Extended Studies (wtcamps@wtamu.edu) with any questions.
All participants that are under the age of 18 will need to have a signature by a parent or legal guardian on all forms. If the participant is 18 years or older, only the signature of the participant is needed on all forms, including those listed below:
Staff Forms
- Access the WTAMU Camp Program for Minors Application via Ideal-logic.
- Upload support documents under the Documents tab of the application, or email them to wtcamps@wtamu.edu.
- Click here for instructions on completing the application.
To report an injury, accident, or safety concern, please use the online Incident Reporting form. Employees, students, and camps, must submit reports within 24 hours of occurrence or discovery.
In case the above electronic portal is not working please forward a PDF copy of the Incident/Injury Report to the Risk Management Office (rcsmith@wtamu.edu) and Extended Studies Office (WTCamps@wtamu.edu).
