Student Absence Notification Request

1. Medical
   • Personal injury or illness that is too severe or contagious for the student to attend  class (an absence for a non-acute medical service does not constitute a reason for an absence notification request.)
   • Illness of a dependent/close family member, such as a spouse, parent, child, grandparent, grandchild, or sibling, or a corresponding in-law or step-relative or others as deemed appropriate.
   • For chronic health conditions resulting in sporadic absences, please register with the Office of Student Accessibility to request accommodations. 
2. Family Death
   • Death or major illness in a student’s immediate family. Immediate family may include a spouse, parent, child, grandparent, grandchild or sibling, or a corresponding in-law or step-relative or others as deemed appropriate. (Students could be granted up to five (5) consecutive class days of bereavement.)
3. Legal or Governmental Proceedings
   • Participation in legal or governmental proceedings that require a student’s presence and that cannot be rescheduled.
4. Professional & Educational Activities
   • Mandatory interviews for graduate or professional school that cannot be rescheduled. 
   • University-sponsored activity that requires mandatory participation.

   • Written confirmation from the student’s medical provider if medical professionals are involved in the care of the student. Confirmation should include the date and time of the medical assessment and the date at which the student may return to classes. Students cannot be required to provide detailed medical information.
   • Written confirmation from the medical provider involved in the care of the student’s close family member or dependent if medical professionals are involved in the care of the family member or dependent. Students cannot be required to provide detailed medical information.
   • For chronic health conditions please reach out to the Office of Student Accessibility for information on what documentation is needed. 
2. Family Death
   • Death notice, obituary, or death certificate for a student’s immediate family member. 
3. Legal or Governmental Proceedings
   • Copy of appropriate documentation related to legal activities.
4. Professional & Educational Activities
   • Confirmation of mandatory interviews from employers or appropriate officials at institutions of higher education.
   • Advisor or Coach should contact faculty on the student’s behalf confirming participation in a University-sponsored activity. 

Faculty will determine how absences will be considered related to the attendance policy for specific class-related activities. Students are advised to consult their course syllabi and contact their individual faculty members (beforehand or upon return) to obtain information on how absences will be handled for each class or to discuss make-up work.  

If a student wishes to contest the fairness of an instructor’s attendance policy, a complaint should be filed per the Student Grievance procedures. When instructors believe that excessive absences have put a student’s grade in jeopardy, the instructors should report this information to Advising Services via Early Alert, who will contact the student and attempt to assist.
