Registrar Proof of Enrollment

Proof of Enrollment

You can request enrollment verification at the Office of the Registrar, by mail,  fax, or email.  If you don't have a form from the inquiring company or if you do not have access to a fax machine or are unable to mail the request, you may send a request via your Buff's student email account containing the following information:

  • Full name
  • Student ID number
  • Last four digits of your social security number
  • Current address and phone number
  • Anticipated graduation date
  • Type of verification
  • Indicate semester and year (e.g., Fall 2017)
  • Verification for (insurance, loan company, bank, scholarship, etc.)?
  • Recipient name
  • Recipient address/fax/email
  • Signature (not required if the request is being sent from your student email account)
