Benefits Health Care Generally, employees can choose between two A&M care plans (PPO's) and FirstCare West Texas (HMO). Under the A&M care plans, you may use any doctor, hospital or other provider and receive benefits. However, you receive higher benefits if you use a provider in the plan's network. The A&M care plans are administered by BlueCross BlueShield of Texas. Other Benefits Dental Insurance
The A&M Dental plan is designed to ensure that you receive good preventive care. The plan covers four types of care: Preventive (checkups, cleanings); Basic (fillings, root canals, extractions, endodontics); Major (crowns, dentures); and Orthodontics (braces). The Delta Dental HMO also covers most preventive, basic, major and orthodontic services. The plan has a schedule that lists your share of the cost of each dental service.
Vision
Provides coverage for eye exams, eyeglass frames and lenses, and contact lenses as well as discounts on some eye surgeries.
Life Insurance
Pays benefits to your beneficiaries if you die or to you if a covered family member dies. Three plans are offered - Basic Life, Optional Life and Dependent Life. Basic Life provides you with $7,500 in life insurance, $5,000 in AD&D coverage and $5,000 in life insurance on your eligible dependent children. If you have system health coverage, you are automatically covered by Basic Life. Optional Life allows you to choose coverage of up to six times your annualized salary up to a maximum coverage of $1,000,000. Dependent Life allows you to cover your spouse and/or children. Optional Accidental Death and Dismemberment (AD&D)
Provides benefits in the event of an accidental injury that results in the death or dismemberment of a covered person.
Long-Term Disability
Provides income if you cannot work due to a disability.
Spending Accounts
Allow you to set money aside to use to reimburse yourself for health care and/or dependent day care expenses. You never pay federal income or Social Security taxes on this money.
